Interagency Resolution Unit

Background
CCF is authorized by law (under powers and duties of CCF, Social Services Law section 483-b) to identify and resolve interagency issues and jurisdictional disputes that impede access to services for children and youth who have complex needs and may require services from multiple agencies.
Role of the Council on Children and Families (CCF)
CCF's Interagency Resolution Unit maintains confidentiality to help to:
- Facilitate individual children, youth and young adults and their families receiving the most appropriate community-based or residential services
- Minimize delays in arranging services or placement
- Resolve interagency disputes that impede service delivery or placement
When to contact CCF's Interagency Resolution Unit?
- After all service options have been explored by working with relevant local, regional and state officials and services cannot be identified or accessed.
- A parent or caregiver feels their child's needs are not being met.
- A child, youth or young adult is in an Emergency Room, medically cleared discharge, and assistance with a discharge plan is needed.

- Individuals (self-referral)
- Parents, Relatives, Caregivers
- Family Court Judges
- School Districts
- Local Departments of Social Services
- State Agencies and their Local or Regional Offices
- Voluntary (Non-profit) Organizations
- County and State Legislators
- Out-of-state Residential Schools and Programs
- Advocates
What is needed to make a referral? It is helpful if a person making a referral provides the following information, if applicable and available:
- Download the CCF Intake Form and the Consent Form to your local computer which is provided in the multiple languages listed below.
- Completed the Intake Form and signed Consent Form.
- Medical History
- Psychological Assessment
- Psychiatric and Functional Assessments
- Psycho-social History
- Individualized Education Program (IEP)
- Tracking and Billing System (TABS) Number
- Court Proceedings
- Recent Service and Placement History
- Description of Barriers Encountered in Accessing Services
